I need to add new custom output columns but I don't have the "manage out columns" link. How do I create new custom columns?

Output columns from what? For example, to create new output columns from a standardization activity, you edit the dictionary (to define those columns) and the pattern action language (to populate them).

This cannot be done for out-of-the-box rule sets. However, you can make a copy of such a rule set and edit the copy.
